Pineal region epidermoid cyst

Presentation

Headache, seizures.

Patient Data

Age: 10 years
Gender: Male
mri

High T2W signal lesion in the pineal region in midline and towards left side, appearing hyperintense on T2W, DWI with incomplete suppression on FLAIR images. The hyperintensity on DWI is due to a combination of T2W shine through and true diffusion restriction. The lesion is causing mild splaying of posterior aspects of third ventricles with mild compression of tectum and splenium of corpus callosum. 

Note is made of cavum septum pellucidum et vergae. 

Case Discussion

Although unusual, the findings are consistent with an epidermoid cyst of the pineal region.
