Large well-defind intrasellar mass enlarging the sellar fossa with suprasellar extension, filling the optochiasmatic cistern and compressing the optic chiasma. The solid non-necrotic component displays an isosignal to the cortical grey matter on both T1 and iso-to-high signal T2 with moderate and heterogeneous enhancement on postcontrast sequences. The necrotic component is of low signal on T1 and high signal on T2. There is extension to the right cavernous sinus (Knops 3A) with a normal diameter of the intracavernous internal carotid artery.

Case Discussion

MRI features of a pituitary macroadenoma with suprasellar extension compressing the optic chiasma.

Large lesions are often heterogeneous and vary in signal due to areas of cystic change, necrosis, and/or hemorrhage.
