Pituitary macroadenoma - cavernous sinus invasion (grade IV)

Presentation

High prolactin level in a non-pregnant female (hyperprolactinemia).

Patient Data

Age: 40 years
Gender: Female

There is a mass arising at the sella turcica with right parasellar extension. It appears of intermediate signal intensity on both T1 and T2 with homogeneous post-contrast enhancement. The mass is totally encircling the cavernous portion of the right internal carotid artery which is attenuated in size when compared to the contralateral side. The pituitary stalk is enhancing and deviated to the left side.

Case Discussion

Considering the given clinical history of hyperprolactinemia, the MR findings are keeping with pituitary macroadenoma with right parasellar extension and grade IV invasion of the right cavernous sinus.
