Presentation

Left heel pain 7 days. No trauma. Windlass test positive

Patient Data

Age: 75 years
Gender: Male

The study reveals a thickening and minimal hypoechoic echoparttern of the left plantar fascia (only proximal part). The left middle and distal plantar fascia were normal (image not uploaded).

The right plantar fascia shows normal thickness and echopattern.

Left plantar calcaneal spurs.

Case Discussion

  • The plantar fascia (PF) is a common problem in the adult problem.
  • Plantar fasciitis is the most common disorder of the PF and a frequent cause of heel pain in the general population.
  • Plantar fasciitis is the inflammation of the plantar fascia.
  • Plain film finding: PR thickening (> 5mm), Fat pad abnormality (narrowed or absent), fine calcification at calcaneal insertion, plantar calcaneal spur at origin.
  • Ultrasound reveals: loss of fibrillar structure, increased thickness over 4 mm, calcification within the PF, perifascial collection and hyperemia. 
  • MRI shows an intermediate signal on T1-weighted sequence and increased signal on fluid-sensitive sequences
  • Differential Diagnostic = Plantar fibromatosis (Ledderhose Disease), Trauma, infection.
