Planum sphenoidale meningioma

Presentation

Head trauma. ?Intracranial hemorrhage

Patient Data

Age: 75 years
Gender: Male

Extra-axial hyperdense lesion of the right side of the planum sphenoidale with related hyperostosis consistent with a meningioma. On the axial images it can be difficult to appreciate that the lesion is extra-axial and in the setting of trauma this might be misinterpreted as being an intra-parenchymal hemorrhage as the inferior frontal lobe is a common location for traumatic hemorrhage. An additional calcified right frontal meningioma is seen more superiorly.     

Case Discussion

Small planum sphenoidale meningioma simulating a right inferior frontal lobe traumatic hemorrhage in an elderly male following head trauma. This case emphasizes the importance of;

  • not assuming all hyperdense intracranial lesions are hemorrhage in the setting of trauma.
  • examining abnormalities in multiple planes to determine intra or extra-axial status.
  • looking for hyperostosis to help support a diagnosis of meningioma.
