Presentation

Right chest pain. Pulmonary embolism?

Patient Data

Age: 65 years
Gender: Female
CT

Chest

Right 8th rib expansile soft tissue mass. No pulmonary embolism. 

Ultrasound

Chest wall

US confirms a hypoechoic solid mass expanding the rib. No internal vascularization. Core-biopsy performed. 

Case Discussion

Microscopy: Both cores consist of tumor comprising sheets of atypical plasma cells. There is moderate variation in nuclear size with an occasional binucleate cell. Immunoperoxidase stains show strong and diffuse tumor cell staining for the plasma cell marker CD-138 as well as kappa light chain. There is no lambda light chain staining.

Diagnosis: Right 8th rib mass core biopsies: Plasmacytoma.
