Presentation

Painless gradually enlarging left parotid swelling since 3 years.

Patient Data

Age: 65 years
Gender: Female

A well-defined encapsulated lobulated lesion involving the superficial lobe of left parotid gland appearing T1 hypointense, predominantly T2 hyperintense with few T2 hypointense foci/septae within and showing heterogeneous post-contrast enhancement. No evidence of extra-parotid extension. Surrounding fat planes are normal. Findings are suggestive of pleomorphic adenoma.

A well-defined macrolobulated solid lesion is noted in the left parotid with internal vascularity on color Doppler. 

Case Discussion

FNAC proven pleomorphic adenoma of left parotid gland.

The principal differential to consider is the Warthin tumor, which appears as multi-focal or bilateral lesions. Lobulated appearance of the lesion favors the diagnosis of the pleomorphic adenoma, while cystic areas are more frequently seen in Warthin tumors than in pleomorphic adenomas 1.
