Case contributed by Henry Atkinson
Diagnosis almost certain

Presentation

Right neck lump.

Patient Data

Age: 25 years
Gender: Male

Ultrasound neck

ultrasound

At the tail of the right parotid gland, there is a well-defined lobulated heterogeneously hypoechoic solid lesion with some posterior acoustic enhancement and predominantly peripheral vascularity.

MRI neck with contrast

mri

Right parotid superficial lobe lobulated lesion with T1W hypointensity; T2W/STIR hyperintensity and enhancement consistent with a pleomorphic adenoma. A radiological differential includes a low-grade parotid neoplasm.

Case Discussion

This right parotid mass demonstrates the classical imaging findings of a salivary gland pleomorphic adenoma, lobulated hypoechoic lesion on US and T1 hypointensity /STIR hyperintensity on MR. Fine needle aspiration was performed and this was confirmed as pleomorphic adenoma on cytohistopathology.
