Pleomorphic adenoma - parotid

Presentation

Progressive painless swelling of the right parotid gland.

Patient Data

Age: 45 years
Gender: Male
Ultrasound

A well-defined, lobulated homogeneous hypoechoic mass within the superficial lobe of the right parotid gland with posterior acoustic enhancement.

 

MRI

The previously described mass shows an iso-signal intensity to the muscles on T1WI, high signal intensity on T2WI with hypointense peripheral fibrous capsule and heterogeneous enhancement following IV contrast. On DWI, it appears of high signal with high ADC (no restricted diffusion). The ADC within the mass is 2.21 x normal (>1.2 usually suggestive of pleomorphic adenoma).

The TIC (time signal intensity curve) shows a progressive and gradual enhancement within the tumor with delayed peak time (283 s) giving the appearance of a TIC type A according to Yabuuchi et al classification. 

The left parotid gland appears normal.

No cervical lymphadenopathy.

Case Discussion

Ultrasound and MR features of pleomorphic adenoma of the right parotid gland.

An FNAC was performed and showed characteristic features of a pleomorphic adenoma.
