Pleomorphic adenoma - recurrence

Case contributed by Dr Ammar Haouimi

Had surgery for a right parotid mass fifteen years ago.

Patient Data

Age: 45 years
Gender: Female

Numerous well-defined nodules within the right parotid gland ranging from 4 to 16 mm of intermediate signal on T1WI, high signal on T2WI with a peripheral hypointense capsule, and heterogeneous enhancement on postcontrast sequences. No restricted diffusion on DWI/ADC. The time signal intensity curve (TIC) shows a progressive and gradual enhancement within the tumor with delayed peak time (243 s) giving the appearance of a TIC type A according to Yabuuchi et al classification.

The ADC within the mass is 2.38 x normal (>1.2 usually suggestive of pleomorphic adenoma).

The left parotid gland shows a normal appearance.

No cervical lymphadenopathy.
