Pleural metastases - thyroid cancer

Diagnosis almost certain

Presentation

Shortness of breath.

Patient Data

Age: 80 years
Gender: Male

Numerous nodular appearing masses expand the pleural cavity and widen the mediastinum. 

Extensive bilateral pleural metastatic disease is noted coating both the parietal and mediastinal surfaces and extending into the pulmonary fissures. Note absence of the thyroid gland. Bilateral low density renal lesions are also noted, probably cysts. 

CT from a few years earlier

Selected images from a CT scan obtained a few years earlier demonstrate a partially calcified mass in continuity with the right lobe of the thyroid extending posteriorly into the superior mediastinum. This was subsequently resected demonstrating thryoid cancer.
