Pneumatization of the petrous temporal bone apices is an anatomical variant that may be symmetrical bilaterally or asymmetrical. It is important to recognize asymmetrical pneumatization as normal anatomical variation and not confuse it with a tumor or pathological lesion 1,2. Benign lesions such as cholesterol granuloma and cholesteatoma are more likely to occur in a pneumatized petrous apex 1,2.
