Presentation

Pneumocephalus after surgery of evacuation of subdural fluid collections.

Patient Data

Age: 80 years
Gender: Male

Axial CT shows compression and separation of the frontal lobes secondary to the presence of gas within the interhemispheric space. Lung windows shows the double-peaked silhouette of the Mount Fuji sign. There are subdural fluid collections.

Case Discussion

Pneumocephalus is a common finding after surgery.  Air accumulation can produces a mass effect on adjacent brain. The air pressure exceeds the surface tension of cerebrospinal fluid between the frontal lobes giving the appearance of Mount Fuji.
