Extra axial air is collection extra axially at both frontal, left temporal and interhemispheric fissure giving the appearance of Mount fuji sign indicating tension pneumocephalus.
Nevertheless, there is intraventricular haemorrhage at lateral and 3rd ventricles more appreciated at the right lateral ventricle with moderate ventricular dilatation, right frontal subdural haematoma and subarachnoid haemorrhage.
Case Discussion
After placement of VP shunt the air introduced to the brain and the patient developed tension pneumocephalus which is neurosurgical emergency.
