A bony opacity is seen anterior to the C6 vertebral body associated with extensive air tracking superiorly (deep and superficial emphysema) and inferiorly (pneumomediastinum) due to perforation of the hypophayrnx/upper esophagus by ingestion of a bone (ox tail, confirmed surgically).
