On lateral projection a small amount of gas outlines the carina, representing pneumomediastinum.

Air-density space can be seen from level of T1 vertebrae around common carotid artery (more obvious on left side); extending down beside trachea to the level of trachea bifurcation.
There is no identifiable perforation of trachea nor esophagus. There is no penetrating thoracic injury.
Case Discussion
In an otherwise will individual without a history of instrumentation or vomiting, asthma is the most common cause of a spontaneous pneumomediastinum. No treatment is required in the vast majority of cases.
