Presentation

Road traffic injury with frontal head trauma. Normal neurological exam.

Patient Data

Age: 30 years.
Gender: Male
ct

There is gas within the left parotid duct.

Displaced fracture of the floor of the left orbit with herniation of the extracolonic fat and the inferior rectus muscle, Left maxillary hemosinus. The infraorbital canal is unremarkable.

Comminuted displaced fracture of the nasal bones.

Subcutaneous parietal hematoma measuring 8 mm.

No intracranial bleeding. No hydrocephalus or brain herniation. No midline shift.

Case Discussion

The pneumoparotid, in this case, is unlikely due to trauma, since there is no mandibular or maxillary fracture, or anywhere around the parotid duct. It is more likely to be due to a self-induced parotid gas insufflation (Valsalva maneuver, cough suppression).
