Presentation

Known history of advanced colorectal carcinoma presenting at the emergency department with distended and painful abdomen.

Selected axial CT image using lung window shows the presence of free intra-abdominal air better.

Case Discussion

This patient had an intestinal perforation due to the advanced neoplastic disease. Unfortunately he died one day after the presentation. 

Pneumoperitoneum is gas within the peritoneal cavity, and often is the harbinger of a critical illness.
