There is a right internal jugular vein central line with tip in the SVC.
There is free gas below the right hemidiaphragm.
Case Discussion
On discussion with the clinical team, the patient had recently had a laparotomy and the free gas was considered normal at this stage post-op.
Pneumoperitoneum is important to recognize as in the abscence of a known iatrogenic cause such as recent surgery or peritoneal dialysis, hollow viscous perforation is the most likely cause.
