Presentation

24 week twin. Intubated. Umbilical catheters. Increased oxygen requirement.

Patient Data

Age: 5 days
Gender: Male
x-ray

ETT, NGT, UAC, UVC. All tubes in appropriate positions.

Lungs are well expanded and clear. No effusion or pneumothorax.

Large pneumoperitoneum.

Confirmation of large volume pneumoperitoneum.

Case Discussion

Large volume pneumoperitoneum. It is vital to review the upper abdomen on neonatal chest films. This is especially true when the neonate is an extreme preterm. In this case, the cause was perforated NEC, which is the dominant cause.
