Pneumothorax and pneumomediastinum on supine chest x-ray

Day 1 post bioprosthetic aortic valve replacement.

Patient Data

Age: 65 years
Gender: Female

Pneumomediastinum noted. Moderate volume right upper zone pneumothorax characterized by loss of lung markings. Deep sulcus sign right side. 

Subsequent pigtail catheter resulted in resolution of symptoms.

Case Discussion

In a supine position, pneumomediastinum and a deep sulcus sign can be characteristic of a pneumothorax.
