Presentation

Abdominal pain.

Patient Data

Age: 60 years
Gender: Female

Multiple thin walled cysts of varying sizes are present throughout the liver without apparent biliary tree dilatation. The giant cyst measures about 90 mm in diameter. 
A 60 mm simple cortical cyst is present at the lower pole of the left kidney.

Additionally, a few small cortical cysts are scattered at the kidneys. 

Case Discussion

Features on MRI are most compatible with polycystic liver disease (PCLD), which is a hereditary condition. 75-90% of patients with autosomal dominant polycystic kidney disease have PCLD, and PCLD without ADPKD has a prevalence of <0.01%.

Most patients are asymptomatic, but occasionally patients can have pain with large cysts.
