Patient has severe pain in bilateral legs with mild swelling, fever and muscle weakness.

Patient Data

Age: 50 years
Gender: Male

Bilateral almost-symmetrical multifocal feathery-edema involving the bilateral posterior muscle compartments, mainly bilateral soleus muscles. Minimal fluid signal is seen between affected muscle compartments.

Case Discussion

The clinician's initial impression was bilateral cellulitis. MRI for both legs was performed and the findings were suggestive of polymyositis.

Differential diagnosis:

  • collagen vascular diseases
  • muscular dystrophy
  • inclusion body myositis
