Presentation
Scrotal discomfort. Mass palpated on physical exam.
Patient Data

Inferior to the normal appearing left testicle in the scrotal sac, there are two additional homogenous, rounded structures. A separate epididymis is seen along the superior aspect of the additional echogenic masses. There is normal colour Doppler flow in the left testis and additional paratesticular masses. There are no echogenic masses within the inguinal canal.
The right testis and hemiscrotum are normal.
Case Discussion
This is a case of polyorchidism with four total testes (3 in the left hemiscrotum and 1 in the right hemiscrotum). As the supernumerary testes appeared normal and the patient did not have significant clinical symptoms, observation and interval ultrasounds were recommended.
