Elderly patient presenting with an acute dense right hemiplegia.

CT - day 1

Initial CT, obtained 24 hours after symptom onset, demonstrates an ill-defined region of low attenuation in the left side of the pons, which is age indeterminate but was thought to potentially represent an infarct.

CT - day 3

Repeat CT 2 days later confirms evolution of a left sided pontine infarct. 

Case Discussion

CT is undervalued in assessing for this sort of stroke. Willingness to repeat imaging the next day can often make the diagnosis. Obviously, MRI is more sensitive and will be positive immediately.
