Popliteal venous aneurysm

Case contributed by Bálint Botz
Diagnosis certain

Presentation

Slowly worsening localized pain and tenderness around the medial aspect of the right ankle. No history of trauma.

Patient Data

Age: 50 years
Gender: Female

US medial aspect of the ankle

ultrasound

The immediate cause of the symptoms is a superficial thrombophlebitis of the distal part of the GSV.

US - popliteal fossa

ultrasound

As a concerning "incidental" finding a circumscribed dilation of the popliteal vein was observed proximal to the SSV junction. Note the marked spontaneous echocontrast indicating turbulent, disturbed blood flow.

Case Discussion

While the superficial and circumscribed thrombophlebitis can be easily resolved using conservative therapy, the asymptomatic popliteal venous aneursym warrants close follow-up, and possibly surgery. Deep venous aneurysms are rare, but life-threatening as they predispose to DVT and pulmonary embolism.
