Post-operative biliary stricture

A 45 year old female was operated for cholelithiasis 3 months back. She had undergone cholecystectomy. Patient returned to the surgical outpatient department with jaundice and abdominal pain, with on and off fever.

Patient Data

Age: 45 years
Gender: Female

MRI and MRCP images reveal an abrupt smooth cut-off of common hepatic duct approximately 1 cm from the hilar confluence. There is proximal dilatation of intrahepatic biliary tree.

Note is also made of a collection in the region of gallbladder fossa, suggesting postoperative biloma.

Case Discussion

A diagnosis of postoperative type II biliary stricture with biloma was made.
