Chest and right hypochondrial pain. Cough.
Patient Data
Bilateral upper lobar and apical lower lobar multiple consildative patches and cavitations showing thick walls, associated with multiple tree-in-bud and nodular densities.
Normal CT scan of the abdomen and pelvis,
Gene Xpert MTB-RIF, sputum was positive for TB
Sputum film for acid-fast bacilli (3 samples), Z.N stained film was positive
Case Discussion
Post-primary tuberculosis occurs in the setting of diminished immunity and usually involves the apical segments of the lung lobes.
Common radiological findings of active pulmonary TB include consolidative patches showing cavitations, ree-in-bud densities secondary to endobronchial spread and lymphadenopathy which occurs in 5% of cases.
