Post splenectomy chest radiograph

Case contributed by Dr Henry Knipe

Post-operative chest x-ray.

Patient Data

Age: 30 years
Gender: Male

Large volume of free subdiaphragmatic gas, absent spleen shadow, and LUQ drain tube - in keeping with splenectomy.

Left ICC tip projects over the mediastinum. No pneumothorax identified. Subcutaneous gas and pneumomediastinum noted.

Case Discussion

There is evidence of trauma with an intercostal catheter and pneumomediastinum. The spleen is a frequently injured abdominal organ in blunt trauma, and in this case the patient had a high-grade injury requiring splenectomy.
