Initial presentation with a 3-month history of right shoulder trauma followed by pain. No past history of a similar injury.

Post-traumatic distal clavicle osteolysis

Irregularity of cortex and loss of bone is seen at the distal clavicle favoring osteolysis. Acromial cortex is normal.

Right AC joint space widening, capsular thickening and effusion.
Left AC joint for comparison is normal.
Case Discussion
In view of the history of trauma and radiograph - ultrasound images, post-traumatic distal clavicle osteolysis is most likely.
Other differentials, which are ruled out, include:
- rheumatoid arthritis
- hyperparathyroidism
- multiple myeloma
- metastasis
- cleidocranial dysplasia
- pyknodysostosis
