Post traumatic internal carotid-internal jugular AV fistula

Presentation

The patient had a stab injury in the left lateral cervical region. Now he complains of throbbing headache.

Patient Data

Age: 45
Gender: Male

A rapidly filling arteriovenous fistula (AVF) between the proximal  left internal carotid artery (just after its origin) and the internal jugular vein.  

Hypertrophied dilated draining internal jugular vein showing early venous opacification with multiple adjacent dilated venous channels.

Hypertrophied patent distal cervical, petrous, cavernous and supraclinoid portions of the left internal carotid artery.  

Small pseudoaneurysm is seen projecting from the posteromedial aspect of the left internal carotid artery opposite the site of the AVF,it measures about 8x9 mm having a wide neck measuring 5mm, No mural thrombosis or contrast extravasation. 

A rapidly filling arteriovenous fistula (AVF) between the proximal  left internal carotid artery (just after its origin) and the internal jugular vein (red arrows). 

Small pseudoaneurysm (P) is seen projecting from the medial aspect of the left internal carotid artery opposite the site of the AVF.
