Post tuberculosis - destroyed lung
History of sputum positive pulmonary tuberculosis 3 years back. Treatment defaulter presented with increasing breathlessness.

Destruction of left lung parenchyma with replacement by a large cavity showing free communication with bronchial tree and air-fluid level within it. Fibrocavitery changes and calcified granulomas in right lung.
In this case left lung has been destroyed by tuberculosis.
It has been found that pulmonary haemodynamics play a part in the pathogenesis and spread of tuberculous infections which causes more frequent involvement of left lung 1.
Pneumonectomy can be considered.
