Posterior cerebral artery (PCA) infarct - cavernoma

Presentation

Weakness of the right upper limb, dizziness and nausea.

Patient Data

Age: 75 years
Gender: Female
  • hyperintense FLAIR and T2 signals with restricted diffusion in the left posterior cerebral artery territory with no hemorrhagic component on GE sequence in keeping with acute infarct

  • cerebral volume loss with small vessel ischemic changes of the supratentorial white matter

  • small well-defined ovoid intra-axial lesion of the left middle cerebellar peduncle of mixed signal intensity centrally on both T1 and T2 with prominent blooming artifact on GE sequence with no surrounding edema in keeping with cavernoma

Case Discussion

MRI features of an acute posterior cerebral artery infarction with cerebellar cavernoma type II according to Zabramski classification of cerebral cavernomas (incidental finding in this case).
