Posterior circulation stroke - right PICA territory

Diagnosis almost certain

Presentation

Headache, confusion and vomiting for 5 days. Right arm weakness. Medical history: diabetes mellitus and hypertension.

Patient Data

Age: 60 years
Gender: Male

High T2 and flair signal with evident diffusion restriction at:

  • the lower part of right cerebellum, extending paramedian superiorly
  • right cerebellar tonsil
  • the posterior right aspect of lower part of medulla oblongata

MR angiography

Marked attenuation of the right posterior inferior cerebellar artery (PICA).

Stenosis of the distal right vertebral artery.

Narrowing of the intracranial segment of the left internal carotid artery (ICA) with multiple plaques.
