Sudden onset unsteady gait, veretigo, diplopia and nystagmus.

Focal area of low signal on T1WI, high signal on T2WI/FLAIR with restricted diffusion involving the posterior inferior aspect of the right cerebellar hemisphere and the lateral medulla oblongata with a subtle leptomeningeal enhancement on post-contrast sequences. Mild mass effect is noted with partial effacement of the cerebellar folia and the Perimedullary cistern.
Case Discussion
MRI features are most consistent with a cerebellar infarction in the posterior inferior cerebellar artery (PICA) territory.
PICA territory is considered the most common territory involved in cerebellar infarction.
