Large area of low signal on T1, high signal on T2 and FLAIR with restricted diffusion involving the left posterior inferior cerebellar artery (PICA) territory with no hemorrhagic component seen on GE sequence.
The MRA (3D-TOF) shows a hypoplastic left vertebral artery with no visualization of the posterior inferior cerebellar artery (PICA) compared to the right side.
Cerebral volume loss with small vessel ischemic disease.
Case Discussion
MRI appearances in keeping with an acute PICA territory infarct.
