Presentation

Dizziness and ataxia. Nausea.

Patient Data

Age: 70 years
Gender: Male

Multiple foci of avid diffusion restriction in the inferomedial aspect of the left cerebellar hemisphere, inferior vermis and the left posterolateral aspect of the medulla oblongata. 

Extensive established periventricular small vessel ischemic change in the cerebral hemispheres.

Case Discussion

The typical history and MR appearances of a posterior inferior cerebellar artery (PICA) infarct.

It illustrates the territories supplied by the PICA artery in its entirety, including part of the medulla oblongata.

Posterior circulation strokes are difficult to appreciate on CT but beautifully illustrated on MR.
