Posterior shoulder dislocation

Case contributed by Dr Ian Bickle

Presentation

Rugby injury.

Patient Data

Age: 35 years
X-ray

The alignment of the humeral head and glenoid is altered with the head of humerus en face on the AP projection with the appearance of a light bulb.

No fracture.

Case Discussion

High energy force to the shoulder during a contact sport resulted in this posterior shoulder dislocation, which was proven in theater at the time of reduction.

The classical light bulb sign is well illustrated.

Posterior dislocations make up a tiny proportion of all shoulder dislocations.
