Presentation
Painful right shoulder with restricted motion. History of trauma to the right shoulder a few years ago.
Patient Data
There is a posterior dislocation of the humeral head with a large bony defect of its anteromedial aspect "reverse Hill-Sachs lesion", impacted against the posterior margin of the glenoid cavity with increased distance between the medial aspect of the humeral head and anterior glenoid margin. There is an associated small fracture of the posterior glenoid margin "reverse Bankart lesion"
Case Discussion
Posterior shoulder dislocations are less common than anterior shoulder dislocations and account for only 2-4% of all shoulder dislocations. Reverse Hill-Sachs lesion and reverse Bankart lesion are common associated injuries (as in this case).
