Posterolateral corner injury

Presentation

Hyperextension injury one week ago.

Patient Data

Age: 30 years
Gender: Male
mri

Fibular collateral ligament is intact but with periligamentous high signal. Biceps femoris partial tear proximal to the conjoint tendon extending into the anterior slips attaching onto lateral tibial condyle where there bone marrow oedema. Fibular head periosteal stripping and bone marrow oedema. Arcuate ligament partial tear. Popliteofibular ligament appears torn. Small bundle of ligamentous tissue superior to the arcuate/popliteofibular footplates at the fibular head. Fibular soleus attachment partial tear. No fabella.

Bone marrow oedema through the anterior medial femoral condyle and anterior medial tibial plateau, with a tiny focus at the anterior lateral tibial plateau.

No knee joint effusion. No meniscal injury. Anterior and posterior cruciate ligaments are intact. Medial collateral ligament is intact.

Case Discussion

The anteromedial femoral condyle and tibial plateau bone marrow oedema in combination with the significant posterolateral corner injury suggests a hyperextension and varus force mechanism of injury.
