Postsurgical ureter injury

Presentation

Complaints: abdominal pain, fever. CT examination followed combined laparoscopic hysterectomy and cholecystectomy with ureteral stenting.

Age: 50 years
Gender: Female

  1. right ureter contrast media extravasation due to postsurgical injury, which was evident after excretory phase prone imaging.

  2. uroperitoneum with right ureter injury.

Case Discussion

Two days after GYN surgery. There was no evident extravasation of contrast agent on supine CT images, but the unusual amount of peritoneal fluid was uncharacteristic of the postoperative state. In the supine position, we do not see extravasation of the right ureter as the loops of the small intestine close the defect on it. Thirty minutes after the delay phase, the scan was repeated in the prone position; a defect of the lower third of the right ureter was identified with contrast media active extravasation, with ascites becoming hyperdense.
