Pott puffy tumor

Presentation

The patient presented with left frontal swelling, fever, and leukocytosis.

Patient Data

Age: 11 years
Gender: Female

Complete opacification of both frontal sinuses with cortical erosions involving the sinus bony walls and frontal bone, in keeping with an aggressive process (osteomyelitis). 

Soft tissue window shows complete opacification of both frontal sinuses and to a lesser degree the ethmoid air cells with associated intra- and extra-cranial near fluid density collections and scalp thickening seen in the left frontal region.

Mucosal thickening of the right maxillary sinus is also noted.

Extra-axial biconvex, peripherally enhancing collection showing restricted diffusion seen in the left frontal region slightly crossing the midline, in keeping with epidural abscess formation.

Another similar extracranial collection is seen in the left frontal scalp area.

Mucosal thickening and variable opacification involve both frontal, anterior ethmoids, and right maxillary sinuses.

The visualized dural venous sinuses look normal.

Case Discussion

Pott puffy tumor is a complication of frontal sinusitis manifested by a triad of frontal sinusitis, osteomyelitis, and subperiosteal abscess formation. It is commonly seen in adolescents.

Treatment includes drainage of abscess with IV antibiotics.

Complications that could develop with pott puffy tumor include meningitis, dural sinus thrombosis, and cerebral abscess formation.
