Presentation

Dorsal and left lower chest pain in patient with fever and night sweats.

Patient Data

Age: 80 years
Gender: Male
mri

Localized gibbus deformity of the mid-dorsal spine with partially collapsed vertebral bodies from T5 to T8 of low signal intensity on T1WI, high signal intensity on STIR with enhancement on postcontrast sequences. Intradiscal abscesses with the intracorporeal extension of the adjacent vertebral end-plates mainly of T5, T6, and T7. Thickening with enhancement of the epidural (epiduritis) and prevertebral soft tissues with abscess formation within the pre-and paravertebral spaces. Mild encysted left pleural effusion with signs of costal osteitis of the left adjacent ribs. The adjacent segment of the spinal cord is not compressed with homogeneous signal intensity on all sequences.

Case Discussion

MRI appearances are highly suggestive of Pott's disease of three contiguous levels from T5-T6 to T7-T8.
