Regular ANC visit at 18 weeks of gestation. No complaints.
Patient Data
Single, live, intrauterine pregnancy of 19 weeks 3 days with regular fetal cardiac activity (FHR-148bpm)
The placenta is posterior.
There is a localized echogenic area measuring 37 x 46 x 33 mm (30.5ml) in between the chorionic membrane and the fetal aspect of the placenta which shows no flow on color Doppler interrogation.
Case Discussion
Collection of blood between the chorionic membrane overlying the placenta and the villous chorion (fetal aspect of the placenta) is term as preplacental hematoma 1. The source of bleeding is possibly from fetal vessels and may be associated with maternal hypertension. They have less clinical significance. However, the patient may present with vaginal bleeding or uterine contraction resulting in abortion, intrauterine growth restriction (IUGR), preterm labor, placental abruption and fetal distress.
On the other hand, retroplacental hemorrhage is behind the placenta, separating placenta from the uterine wall.
