Prepatellar bursitis

Presentation

Acute onset of pain and swelling of the right knee. Negative trauma.

Patient Data

Age: 40 years
Gender: Male
x-ray

There is minimal medial joint space narrowing and no other significant degenerative changes. There is prepatellar soft tissue swelling. Suprapatellar effusion is present. Hoffa's fat pad appears normal. Incidental quadriceps enthesopathy.

ultrasound

A small suprapatellar effusion is present. There is a complex, septate, collection anterior to the patella. Hypervascular synovium is demonstrated.

Case Discussion

Features consistent with prepatellar bursitis. The patient's uric acid, inflammatory markers, and white cell count were normal. Gout and infection were considered unlikely. Further rheumatoid arthritis workup was planned. Occupational history appeared negative as a potential source of trauma or irritation.
