Prepatellar gout on dual energy CT

Presentation

Right knee atraumatic swelling and pain. Limited ROM. Past history of gout but always in feet, never in knees.

Patient Data

Age: 80 years
Gender: Male
X-ray

Subcutaneous edema around the knee including mild thickening of the pre-patellar soft-tissues. No joint effusion. No obvious high density material in the soft-tissue to indicate gout.  

Dual energy CT demonstrates monosodium urate deposition superficial to the patella and proximal patella tendon. Marked subcutaneous edema. 

Case Discussion

Acute knee inflammation due to gout (urate crystals) of the prepatellar tissues/bursa.
