Pressure ulcer

Diagnosis almost certain

Presentation

Paraplegia in a patient with a history of traumatic spinal cord injury 10 years ago .

Patient Data

Age: 50 years
Gender: Male
mri

The MRI sequences demonstrate:

  • on the left: loss of soft tissue thickness with deep ulcer cavity of the buttock extending up to the ischial tuberosity which appears thickened of low signal on both T1 and T2 with cortical erosions (osteomyelitis). Associated edematous infiltration with enhancement surrounding the ulcer cavity

  • on the right: loss of soft tissue thickness with deep ulcer cavity extending up to the ischial tuberosity which shows a normal cortical thickness and bone marrow signal

  • atrophied gluteal muscles with fatty degeneration

  • reactive left inguinal lymphadenopathy

  • right hydrocele is noted
