Primary cutaneous melanoma of foot

Presentation

Nodule involving sole of foot - 1 yr.

Patient Data

Age: about 18 - 20 yrs
Gender: Male
Modality: X-ray

A soft tissue density is noted involving sole. No calcification.

No bony abnormality.

Biopsy of the lesion was done. 2 differencials - Anaplastic ca, amelanotic melanoma. Referred for chest radiograph.

Modality: X-ray

Both lung fields are clear.

No abnormality of soft tissue or bony thorax.

Biopsy of the lesion was done. 2 differencials - Anaplastic ca, amelanotic melanoma. Referred for Ultrasound abdomen - pelvis and inguinal regions.

Modality: Ultrasound

LEFT CALF
- three subcutaneous lesions are noted
- Two hypoechoic nodules seen lateral to short sephanous vein - 4 - 5 mm
- A hypoechoic nodules seen medial to short sephanous vein - 3 mm.       

LEFT popliteal fossa        

- An ovoid lesion 
  21 x 17 mm 
  Loss of hilar fat and central vasculariy.

LEFT inguinal region

- Three nodes / lesions are noted.
- Largest lesion -  node  
   -- 19 x 16 mm 
   - Loss of hilar fat and central vasculariy.

- Another node - asymmetric cortical thickening.

Abdomen and pelvis

NO hepatic lesions.

Case Discussion

Excision of subcutaneous lesions in calf, popliteal nodes and inguinal nodes was done.

Histopathology - Anaplastic ca  / melanoma.

IHC study  - Melanoma.
