Primary hydatid cysts - soft tissues

Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Progressive painless swelling of the posterior aspect of the left thigh.

Patient Data

Age: 55 years
Gender: Female
ultrasound

There is a large well-defined lobulated cystic mass of intermuscular location with a relatively homogeneous echotexture of the cystic content.

On MRI this cystic mass appears well-circumscribed multilobulated measuring about (16 x 12 x 9 cm) of intermuscular location well-visualized on sagittal/coronal sequences. It elicits a high signal on both T1/T2 with tiny daughter cysts of a higher signal on STIR. The postcontrast sequences show a regular peripheral enhancement. Other small variable-sized cystic masses of similar signal and enhancement are noted proximal to the previous cystic mass and of intra/intermuscular location.

Case Discussion

Ultrasound and MRI features of hydatid cysts of soft tissues.

The chest x-ray and abdominopelvic ultrasound (not shown) were normal in this case.
