Primary synovial osteochondromatosis

Presentation

Shoulder pain.

Patient Data

Age: 45 years
Gender: Male

There is a large calcific mass surrounding the glenohumeral joint, made up of numerous uniformly sized densities which each have a cortical rim. There are multiple associated pressure erosions on the surrounding osseous structures, most notably the humeral head and surgical neck of the humerus. 

Case Discussion

This is an extreme example of primary synovial osteochondromatosis. A differential consideration in this case would be tumoral calcinosis.
